Incident Reporting

A mature safety management system requires the means to measure safety performance in order to identify trends and make targeted improvements. As part of this performance review the ability to collate safety non-conformances is vital.

Non Conformances 

Non Conformances are deviations to the standards, where drivers behave in a manner that is not correct, or appropriate or do something wrong. 

These will be reported to us at P&O Ferrymasters in various ways – either by the customer, or by one of our Health and Safety personnel on site carrying out auditing. 

These will be sent onto the haulier responsible.  We expect the non conformance to be investigated with the driver and a formal response sent back to us within 48 hours. 

 

Incidents 

These are more serious events, such as an injury or a load shift/loss.  To enable us to prevent the same incident from occurring again, we investigate thoroughly all the events and tasks leading up to the incident. 

For us to do this properly, we need as much information as possible from the driver involved.  This includes driver statements, photographs of the scene, vehicles involved, time, weather conditions, other adverse conditions which might have contributed to the incident.
